SELPHYL- a natural dermal filler in skin rejuvenation

What is the Selphyl process?

The Selphyl System is a new approach to anti-aging treatment which allows a trained physician to prepare a Platelet-rich Fibrin Matrix to use as a dermal filler using blood collected from the patient during an office visit. Using a centrifuge and a chemical catalyst, the doctor is able to use only the platelets and fibrin in facial rejuvenation.

How is PRFM different from PRP?

PRP, by definition is platelets and plasma, where the platelet concentration is generally considered to be twice the normal. PRP should have little to no red blood cells and white blood cells. By adding the precise amount of calcium chloride, the SELPHYL® System can convert the liquid PRP into PRFM, a gel-like matrix. This occurs because the calcium chloride initiates the conversion of fibrinogen to fibrin, as part of the clotting cascade. The fibrin matrix serves as a three dimensional scaffold to maintain the platelets at the site of injection.
The main advantages and benefits of PFRM include:
  • High degree of purity
  • Reproducibility of the treatment
  • Confidence in knowing exactly what is being injected
  • Safety profile

What makes the Selphyl System work?

Selphyl is a natural autologous preparation which the body readily accepts. When injected just under the skin, it activates new skin cell growth and, over time, leads to a stronger collagen support . Selphyl prompts new cells to grow and spread throughout the treatment area.

Selphyl is Platelet Rich Plasma

What areas of the body can be treated successfully with Selphyl?

Selphyl can be used to give a fuller appearance to the face or hands. It can diminish the scars left by acne and greatly improve the lines around the mouth and nose which develop with time. Selphyl may be used on any area of the body which needs to be plumped or to create areas with more definition.

What are the benefits of Selphyl compared to other similar treatments?

The fact that Selphyl is created from the patient’s own body is the biggest advantage this system offers. This means allergic reactions almost never occur. The Selphyl process is finished in twenty minutes or less and is rarely uncomfortable. Selphyl stays fluid under the skin so there is never a need to be concerned about unsightly knots or clumping. The entire Selphyl process takes place in a doctor’s office and requires no general anesthesia. Because Selphyl does not burn or blister the face, infection is unlikely.

Who can be treated with Selphyl?

Patients of all ethnicities are candidates for Selphyl treatment. Doctors do feel that the Selphyl system should not be used on anyone already dealing with blood clotting issues or those who are presently on blood thinning medications.

What happens at an appointment for Selphyl treatment?

HOW SELPHYL® WORKSFirst, blood will be drawn from the patient’s vein. Only 9cc of blood is needed to provide Selphyl for each treatment. After using the patient’s blood to prepare the filler, the aesthetic physician will inject Selphyl into the treatment area. It is placed just under the skin, but some doctors also choose to use a local anesthetic to prevent any discomfort.

What is the recovery after Selphyl treatment like?

Recovery after Selphyl treatment is relatively easy. Pain medication is not required, and the patient will only experience mild swelling and bruising for a day or two. A few may also have minor itching, but this is only a mild irritation.

What will I see after Selphyl treatment?

After two to three weeks, the treated area will look smoother as new cells fill in under the epidermis. The improvement seen will continue for several weeks until signs of aging are greatly diminished.

Is the Selphyl System approved for use in the U.S.?

The FDA approved Selphyl for use by trained providers in the United States, and it has also been certified as safe in Europe.

Is Selphyl treatment covered by insurance companies?

Major insurance companies consider Selphyl treatment to be cosmetic and refuse coverage on this basis. Providers do sometimes agree to stretch payments over an extended period of time. It is best to ask about financing before the first Selphyl treatment

How is SELPHYL® different than the “Vampire Facelift”?

SELPHYL® is not affiliated in any way with the term “Vampire Facelift,” that was coined by the media to describe PRP treatments. The Vampire FaceLift® Procedure™ is a specific technique that combines hyaluronic acid fillers (i.e., Restylane® and Juvederm®) with “blood-derived growth factors” from another brand of PRP. This procedure has been trademarked by Charles Runels, M.D.

What clinical data has been published about the science of PRFM?

Clinical studies published to date include:
Sclafani AP. Platelet-rich Fibrin Matrix for improvement of deep nasolabial folds, Journal of Cosmetic Dermatology. 2010;9.
Sclafani, AP; McCormick, SA, Induction of Dermal Collagenesis, Angiogenesis, and Adipogenesis in Human Skin by Injection of Platelet-rich Fibrin Matrix, Archives of Facial Plastic Surgery, 2011:10;

ARTEFILL Dermal Filler

Artefill is a collagen filler mixed with microspheres used to correct deep nasolabial folds, smile lines, marionette lines, cheek augmentation or to replace volume lost with aging or acute weight loss in your face.
Other dermal fillers are temporarry being eventually absorbed by your body. Artefill has better longevity than hyaluronic acids fillers and require much less repeated maintenance treatments to maintain  the original results.
Artefill is different from temporary dermal fillers. The unique microspheres contained within Artefill are not absorbed by the body, which supports long-lasting, beautiful results. In the U.S., thousands of people have been treated with Artefill and are happy with their results.
How does Artefill work compared to other Fillers?
Artefill works in more than one way to fill wrinkles. Initially, the collagen portion of the product fills in the wrinkle for an immediate result. The microspheres in Artefill continue to fill wrinkles once the collagen has been absorbed in the body.
The procedure can be completed in 15-30 min in a medical office . You will be administered a topical anesthetic to reduce the pain from the procedureonce completed, you can quickly return to your normal daily activities. The collagen provides immediate results, which are supported and fully visible over time.
Is Artefill safe?
Yes. Artefill was approved by the U.S. FDA in October 2006 to be used as a safe and successful dermal filler when used to correct wrinkles. Artefill is safe, as shown in a clinical test where it performed as high in safety as the control. Artefill has never been recalled since it was launched in 2007.
How is Artefill different from other fillers?
Artefill contains a unique combination of collagen and microspheres that work together to maintain long-term wrinkle correction results, whereas temporary dermal fillers are made of synthetic or natural materials that are gradually absorbed by the body, yielding wrinkle correction results that last only 3-9 months. Wrinkles correction is maintained with Artefill for a longer period of time due to the microspheres that remain at the injection site even after the collagen is absorbed.
What should I expect?
Immediate results are expected, and studies performed in the U.S. verify that wrinkle correction is maintained for up to one year.
How many treatments are required?
Usually, desired results are achieved within one to two treatments. Decisions about additional treatments can be made during follow-up appointments with your physician in the weeks following your initial treatment.
Who is not a good candidate for Artefill?
Artefill may not be the best choice if you:
· have a positive reaction to the Artefill skin test administered before treatment
· have severe allergies or drug-related allergies
· tend to have thick scarring
· are undergoing desensitization injections to meat products
· have active skin outbreaks near the treatment area
· have any blood-related medical condition that may increase bleeding at the treatment area
· have had other filler treatments to the area recently
· are undergoing UV light therapy
· are currently on immunosuppressive therapy
All individuals should discuss current health conditions and medications with their physician prior to treatment.
Are there any other precautions with Artefill?
Yes, you should know that:
  • The microspheres found in Artefill remain in the skin and can only be removed surgically
  • Artefill is has not been tested for safety in pregnant or breastfeeding women, or in women under 18 years of age
What side effects are associated with Artefill?
As with any injectable wrinkle filler, mild swelling and erythema at the treatment site can be expected. Usually these side effects only last 24-48 hours. There could also be minor bruising that may last 3-7 days. Other side effects, which occur less frequently, include rash and itching, and increased sensitivity at some or all injection sites. On rare occasion, any dermal filler may cause lumps and granulomas.
What are the long-term results of Artefill?
A 90% satisfaction rating was given by patients surveyed 5 years after their first treatment regarding their results. Studies have been performed to evaluate the long-term results of Artefill treatments, which concluded that the patients maintained a natural facial appearance and that the correction of wrinkles was maintained in some cases up to 5 years.
Is Artefill covered by insurance companies?

No, Artefill is considered a cosmetic procedure and the cost is not typically covered by medical insurance companies.

Retinol and Retinoic acid Dermatologist recommended

Lord Frederick Leighton - Venus Disrobing for the Bath, 1866

Derivatives of vitamin A, such as tretinoin and retinol, work to increase cell turnover and exfoliation by stimulating cell production underneath the skin.

This process slows down with age, allowing layers of dead, thick, sun damaged skin cells to remain on the surface. As a result - skin looks dull and thick, pores look large and sun-related damage such as age spots and blotchiness appear.

Retinoic acid speeds up the natural turn over of the skin maintaining epidermis fresh, glowing and youthful looking.

Orchid Aesthetics recommends the following Retinol and Retinoic acid products available in Dr. Kerns Skin care line:

1. TRETINOL 0.5% SERUM: USES PATENTED FORMS OF RETINOL combined with Votamin E and Vitamin C; a poweful antioxidant serum to combat the aging process fight wrinkles, sundamages skin and photo aging.
2. RETINOL 10X SERUM : Retinol mixed with Vitamin C and Co-Enzyme Q10, promotes new collagen and elastin, decreases apperance of wrinkles and pores.
3. RETINOIC ACID CREAMS 0.025%, 0.05% AND 0.1%: Retinoic acids are considered the gold standard for anti-aging therapy. Retinoic acid penetrates deep into the dermis , stimulates cell renewal and reduces the signes of premature aging. Recommended for acne as well as hyperpigmentation.

Retinol (Vitamin A Alcohol)

This is vitamin A. Retinol does markedly stimulate skin repair in immune-deficient rats and rabbits.
Because the human immune system decreases in potency with age, retinol may help skin repair in humans over age 45 or when declines in immune system effectiveness first become significant.
Retinol decreases excessive breakdown of collagen and elastin in the skin. However, it has long been established that excessive retinol on the skin can cause acne in younger persons.
While there is no defined age when retinol ceases to cause acne, in general, those under 25 are more likely to develop retinol-produced acne, while in most people over 45, retinol appears to have very positive actions on skin renewal.

RETINOIC ACID

The typical strength of topical tretinoin creams is 0.025 - 0.1 percent.
Tretinoin is most commonly used as a form of acne treatment. It was the first retinoid developed for this type of topical use.
  • Tretinoin is the best studied retinoid in the treatment of photoaging.
  • It is used by some as a hair loss treatment and also a component of many commercial products that are advertised as being able to slow skin aging or remove wrinkles.
  • Tretinoin is also used to treat and reduce the appearance of stretch marks by increasing collagen production in the dermis.

SIDE EFFECTS OF RETINOIC ACIDS

  • When used, dryness or increased sensitivity to sunlight of the affected skin may occur.More sensitive patients may also experience redness, scaling, itching, and burning.
  • A gradual increase in the frequency and amount of tretinoin application is best, as this allows one's skin to adequately adjust to the drug.
  • Patients should be careful to follow their physician's recommendations when beginning a round of treatment.
  • This product increases the risk of sunburn;care should be taken (shade, sun protectors etc.) to protect treated skin from overexposure to ultraviolet light.
  • Because usage of tretinoin may cause thinning of the skin, it is strongly recommended that patients who are using the drug abstain from hair removal waxing. The wax will, when removed, pull off the top level of epithelium .
  • Women who are or may be pregnant, or who are seeking to become pregnant, are warned against using it SINCE IT HAS TERATOGENIC EFFECTS.
